I recognize there are risks associated with SCUBA diving, snorkeling and underwater photography and indemnify and hold the Government of the Turks & Caicos Islands, its elected and appointed officials, its Tourist Board, sponsors, employees, agents and representatives blameless in regard to any claim whatsoever made by me or by any other person(s), firm(s) or corporation(s) in regard to my participation in the Competition.

I am paying herewith the Entry Fee of Thirty American Dollars ($30.00 U.S.) and agree that if I should submit this or any other form or document associated with the Competition by e-mail or fax, my signature will be treated as if it was my original signature written in pen.
